Usual Dosing (Adults)

100mg q12h , or 200mg q24h

Renal Dosing

[>30 ml/min]: No changes
[15-30]: 100 mg q18h or 50mg q12h.
[<15]: 100 mg q24h or avoid use.

Hemodialysis

Avoid use or give 100mg q24h (on dialysis days give dose after dialysis).
