Usual Dosing (Adults)
Systemic infection: 250-500mg po q6h

Renal Dosing
dialysis [>50 ml/min]: usual dose q8-12h

[10-50]: usual dose q12-24h.

[<10]: q24h (recommend avoid use)

Hemodialysis
dialysis Hemo: Usual dose q24h (recommend avoid use). Supplemental dose not needed.
