Usual Dosing (Adults)

HIV infection:
>60kg: 40mg orally q12h
<60kg: 30mg orally q12h.

Renal Dosing

[CRCL >50]: No changes

[26-50]:
>60 kg: 20 mg q12h.
<60 kg: 15 mg q12h.

[10-25]:
>60 kg: 20mg q24h.
<60kg: 15 mg q24h.

[<10 – not on hemodialysis]: Insufficient data exists

Hemodialysis

Hemodialysis:
>60 kg: 20mg q24h (on dialysis days, schedule dose after dialysis)

<60kg: 15 mg q24h. (on dialysis days, schedule dose after dialysis)
