Usual Dosing (Adults)

Usual initial dose: 10 mg once daily (range: 5 to 20 mg). Reserve higher initial dose for patients with severe hypercholesteremia or patients with homozygous familial hypercholesterolemia. May titrate dose after 2 weeks (increase by 5-10 mg once daily). Dosing range: 5-40 mg/day (maximum dose: 40 mg once daily).

Renal Dosing

dialysis [>30  mL/min/1.73 m2 ]: No changes.

[<30 mL/min/1.73 m2 - Not on hemodialysis]: Start at 5 mg qd. Do not exceed 10 mg qd.

Hemodialysis

dialysis Insufficient data. (Steady-state concentrations of rosuvastatin in patients on chronic hemodialysis were ~ 50% greater compared to healthy volunteer subjects with normal renal function).
