Ranitidine Zantac ® - Renal dosing

Usual Dosing (Adults)

300mg po qhs or 150mg po bid.
Maintenance: 150mg po qhs.
Erosive esophagitis: 150mg po q6h.
(IV/IM): 50mg q6-8h. Continuous infusion: 6.25mg/hr (higher doses are possible).

Renal Dosing

[>50 ml/min]: No changes.

[0 – 49]: 150mg po q24h or 50mg IV q18-24h. If needed, interval may be increased to q12h if patient is carefully monitored.

Hemodialysis

Hemodialysis:
150mg po q24h or 50mg IV q18-24h. Schedule doses after dialysis on dialysis days.
