Usual Dosing (Adults)

UTI: 200 mg qd x 7 days.
Skin and soft tissue infections: 200 mg q12h x 7 days.
Uncomplicated pyelonephritis: 400 mg q12h x 14 days
Pharyngitis/tonsillitis: 200 mg q12h x 10 days
Sinusitis/Upper/lower respiratory tract infection: 400 mg q12h.

Renal Dosing

dialysis [CRCL >50 ml/min]: Administer usual dose.

[10-49]: Administer 50% of usual dose at usual interval or usual dose given half as often.
[<10 ]: Administer usual dose every 3-5 days.

Hemodialysis

dialysis Dose as for CrCl<10, on dialysis days give dose after dialysis
