Levetiracetam - Keppra ® - Renal Dosing

Usual Dosing (Adults)

Partial onset seizures (adjunctive): Initially, 500 mg po bid. May increase q2weeks by 500 mg/dose to a max of 1500 mg bid.

Renal Dosing

[CRCL >80 ml/min]: 500-1500 mg q12h

[50 - 80]: 500-1000 mg q12h

[30 - 50]: 250-750 mg q12h

[<30 ml/min]: 250-500 mg q12h

Hemodialysis

500-1000 mg q24h. Supplemental dose of 250 to 500 mg following dialysis is recommended.
