Famciclovir - Famvir ® - Renal Dosing

Usual Dosing (Adults)
Herpes Zoster: 500mg po q8h x 7 days.
Recurrent herpes simplex (genital): 125 mg po bid x 5 days.
Primary Genital herpes simplex: 250 mg po tid x 5-10 days.
Genital-chronic suppression: 250 mg po bid.

Renal Dosing
dialysis [>60 ml/min] No change

[40-59] Zoster: 500mg q12h. No changes to other regimens.

[20-39]: Zoster: 500mg q24h. Recurrent GH: 125mg q24h. GH suppression: 125mg q12h. 

[<20]: Zoster: 250mg q24h. Recurrent GH: 125mg q24h. GH suppression: 125mg q24h. 

Hemodialysis
dialysis Recurrent Genital herpes or suppression: 125mg after each dialysis.
Zoster or genitial herpes in HIV pt: 250mg after each dialysis.
