Disopyramide - Norpace ® - Renal Dosing

Usual Dosing (Adults)

<50 kg: 100 mg q6h or 200 mg q12h (controlled release)

>50 kg: 150 mg q6h or 300 mg q12h (controlled release); if no response, may increase to 200 mg q6h; maximum dose required for patients with severe refractory ventricular tachycardia is 400 mg q6h.

Renal Dosing

dialysis [>40 ml/min]: No changes
[30-40]: 100 mg q8h
[15-30]: 100mg q12h
[<15 ml/min]: 100mg q24h

Alternative:
[30-40 ml/min]: Reduce dose 50%
[15-30 ml/min]: Reduce dose 75%

Hemodialysis

dialysis 100mg q24h (Not dialyzable)
