Pyelonephritis
Pyelonephritis
Duration of therapy: Usually for 14 days.  If recurrent or failure after 14 days, treat for 6 weeks.		 Possible therapeutic alternatives

 
Uncomplicated
(Outpatient treatment).


Therapy length:
14 days		 Common pathogens
Ciprofloxacin 500mg orally twice daily or  .    
Norfloxacin 400mg orally twice daily or
Ofloxacin 200- 400mg orally twice daily or 
Levofloxacin 500mg orally once daily for 14 days
Augmentin 875mg orally twice daily or 500mg three times daily  or 
Hospitalized patient (recommend IV therapy until afebrile. Usually 24-48h, then switch to oral therapy Common pathogens
[Ampicillin 1 gram IV every 6 hours + gentamicin IV]   or   
Ciprofloxacin 200 to 400mg IV every 12 hours  or
 Levofloxacin 250-500mg IV once daily or Ofloxacin 400mg IV every 12 hours. or 
Ceftriaxone 2 grams IV once daily for 14 days or
Ampicillin-sulbactam 1.5 to 3 grams IV every 6 hours or Piperacillin-tazobactam 3.375 grams IV q6h for 14 days.
