Outpatient treatment: oral tx. 


Hospitalized patients:
IV Therapy
Common pathogens
[ (Bactrim DS bid or Ciprofloxacin 500mg po bid or Levofloxacin 500mg po qd) ] + Metronidazole 500mg po q6-8h
Augmentin 875 mg po bid or 500mg po tid 
Cefotetan 1-2 ivpb q12h  or Cefoxitin 1-2 grams IV q6h
Piperacillin-tazobactam (Zosyn) 3.375 gm IV q6h or 4.5 gm IV q8h
Ampicillin-sulbactam (Unasyn) 3.0 gm IV q6h
Third or Forth generation cephalosporin (e.g. Ceftriaxone 1 gram IV q12h or Cefotaxime 2 grams IV every 4 to 8 hours or Cefepime 1-2 grams q 12h] or Fluoroquinolone (e.g Levofloxacin 500mg IV qd.or Ciprofloxacin 400mg IV q12h, others)  
plus
Metronidazole 500mg po q6-8h
Clindamycin + gentamicin
